CS4EDU Workshop Reimbursement Procedure

Limited financial support is available for travel and lodging (primarily intended for high school teachers). If you requested financial support on your workshop registration, please follow the instructions below to receive reimbursement for your travel and/or lodging.

Instructions for Reimbursement

• Complete this Payee Certification Form.
  • Fill in all of Section A (Payee Information).
  • Sign and date Section C (Payee Certification).

• Collect original receipts for costs you would like reimbursed.

• For International Participants:
  • Submit copy of passport picture page.
  • Submit copy of I-94.
  • For those with B1/B2 or WB/WT visa, sign Compliance Statement.
  • For international addresses, complete the Request for Wire Transfers and Foreign Drafts form.

• For Permanent Residents:
  • Submit copy of permanent residence card (green card).

• Mail Payee Certification Form, original receipts, and any additional required documents to:
